The Commander Home Monitor is an interactive home monitoring device for disease states such as CHF, Diabetes, COPD, Asthma and Hypertension. The modular design allows you to select the Cardiocom peripheral devices that provide the most appropriate and cost-effective care for your patients. Each day your patients use their Commander Home Monitor to obtain objective biometric data such as: weight, blood pressure, blood glucose, peak flow, and oxygen saturation. Patients also answer a series of questions about their current symptoms. Questions are both displayed in large font and spoken. The data is transmitted over the patient's telephone line directly to the Commander Data Management System at your facility.

Cellular Option Now Available

For individuals who do not have a standard telephone line, Commander Cellular includes an integrated cellular modem and uses GPRS technology to transmit objective health data to Cardiocom's Omnivisor® disease management software system.

• The Glucose Meter can store and transmit multiple measurements to the Commander Data Management System at your facility. This data allows you to identify trends, and track historical readings to ensure your patient's blood glucose is within their ideal range.

• The Blood Pressure Monitor allows the patient to measure their blood pressure and heart rate. Blood Pressure monitoring may be helpful in the detection and treatment of hypertension and when titrating medications.

• The Pulse Oximeter provides immediate, reliable readings of oxygen saturation levels. The Pulse Oximeter is an accurate tool offering information to determine the interventions needed for patients with asthma and COPD.

• The Medical Scale is a precision electronic scale that is accurate and consistent to ± 0.1 lb and has a maximum capacity of 500 lbs. The Medical Scale provides current weight and variance from dry weight. The daily weight is transmitted to the Commander Data Management System to provide immediate feedback on the patient's status.

• The Peak Flow Meter assists your patients in being proactive in the management of their asthma and COPD with the easy-to-use, interactive monitor. More than a peak flow meter, the monitor stores FEV1 and PEFR measurements. Multiple measurements can be stored and transmitted to the Commander Data Management System at your facility.

The Commander Data Management System is located at your facility and receives information daily about patients' vital signs and symptoms. Using intuitive software, the System automatically identifies "exception" patients with symptoms outside the parameters set by the patient's healthcare provider. This "management by exception" allows early intervention, including care plan adjustment, to improve patient care and avoid unnecessary hospitalizations. Since the database resides at your site, you have complete and immediate access to all of your patient information, including a broad base of patient and population reports for you to assess enrollment and outcomes.
