In-house, Direct Telemanagement Tools
Cardiocom's OMNIVISOR™ patient management hardware and software system and COMMANDER, TELESCALE® or CARESTAR® remote patient monitoring devices your in-house disease management team with interactive tools for objective daily monitoring. With this system, one case manager can telemonitor significantly more members because the software alerts the case manager only to the members who need attention. Cardiocom customers generally experience a daily exception rate of approximately 15%. This means that if you have 100 members using TELESCALE®, your team will know exactly which 15 members need a follow-up call that day and why. RESPIRATORY-PLAN™ is a fully outsourced respiratory telemonitoring program focused on high-risk COPD and Asthma members operated by Cardiocom Multi-Disease Management nurse specialists.
