Comprehensive Disease Management Program for Biometric and Symptomatic Telemonitoring Data

OMNIVISOR® is a 4th generation disease management hardware and software system that enables case or disease managers and clinicians at your facility to receive information about patients' daily vital signs and symptoms.

Each day, your patients have their vital signs remotely monitored by a "Health Check" at home using their COMMANDER or TELESCALE® device. Daily biometric weight and symptom data are transmitted and analyzed by the OMNIVISOR® System in your office. OMNIVISOR® automatically identifies and alerts you about "exception" patients with symptoms outside of the parameters set by your clinical management team or the patient's physician. Cardiocom's "management by exception" approach lets you know when the patient is outside of the established parameters, thereby allowing you to focus your attention only on patients who need an adjustment to their care plan. OMNIVISOR® allows the case manager to assist the treating physician by providing current, objective, biometric information about a patient. Management adjustments can be initiated in a timely manner, thereby preventing further deterioration of the patient's condition.

The OMNIVISOR® software, developed by Cardiocom, provides carefully designed clinical algorithms that allow the nurse to better manage patients by sophisticated rules-driven exception alerts. Instead of presenting basic risk-stratified "exceptions" or "alerts" for nurse management based on experience, Cardiocom nurses follow a structured, stepwise process of rules-based clinical triage.

The OMNIVISOR® patient management system enables clinical staff to manage patients "by exception". Using the patient data transmitted from Cardiocom's telehealth devices, the OMNIVISOR® program provides detailed analysis of daily vital signs and symptoms. Daily biometric data is trended over time and displayed graphically for easy viewing. In addition to capturing daily biometric and symptomatic data and managing acute alerts, the program integrates health check and care plan compliance questions into the clinical profile for each patient.

OMNIVISOR'S powerful clinical management system also includes patient demographics, healthcare and emergency contacts, medications, blood chemistries, functional labs, clinical notes, daily objective monitoring parameters and symptom data. All monitoring parameters are fully programmable. The system can be customized to integrate and exchange data with most customers' central data warehouse or clinical management program.


Detailed, Objective Reports

Cardiocom puts you in control with detailed and trended "Exception Reports" and other relevant data (e.g., blood chemistries and current medication regimens), which are printed directly at your site. A patient's primary clinician can then determine the best course of treatment. Reports can also be sent via email.

In addition to generating standard Exception Reports, triggered only when the patient data exceeds established parameters, clinicians may request a variety of reports and updates on patients whose vital signs are unstable, have recently been hospitalized or are undergoing medication titration. These reports may include symptoms, weights, blood chemistries, medication regimens, responses to medication changes, etc.

It provides patient population reports of symptoms, hospitalizations and emergency room visits. You have complete and immediate access to all of your patient information.

Easy To Install and Maintain

OMNIVISOR® brings the next generation in enterprise level remote patient monitoring and more cost-effective solutions to Cardiocom's customers. OMNIVISOR® is an ASP.NET® browser-based system built on Microsoft® technologies, utilizing the .NET® 2.0 Framework and SQL Server 2000. OMNIVISOR® can be installed in about a day. Installation and comprehensive training is performed on-site. All hardware and software is provided and serviced by Cardiocom.
