			VNA of Hanover & Spring Grove Selects Cardiocom Telehealth Technology as Its New Telehealth Provider<br>

              VNA of Hanover & Spring Grove, who recently celebrated its 100th anniversary, announced that it has entered into a business relationship with Cardiocom® to provide advanced home telehealth products and services to patients with chronic diseases.  Cardiocom, the Experts in Telehealth(SM), is world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.</p>
            <p>CCardiocom’s telehealth system provides a unique opportunity to proactively manage and customize patient care.  Every day, VNA of Hanover & Spring Grove patients use the Cardiocom Commander Telehealth System to communicate with a VNA telehealth nurse.  The Commander Home Telemonitoring System guides the patient through an interactive Health Check and collection of vital signs including: weight, blood pressure, oxygen saturation and heart rate.  This information is immediately available for review via a standard phone line to the agency’s telehealth department.</p>
