COPD and Asthma are respiratory diseases that can be effectively managed with close daily symptom telemonitoring and appropriate pharmacotherapy. Our experienced Respiratory Nurses use the CARESTAR to telemonitor data daily from your patients’ home and communicate any out of bounds symptoms to the managing physician. RESPIRATORY-PLAN is unique from other DSM programs in that the Cardiocom CARESTAR obtains symptom data EVERY SINGLE DAY. The key to the RESPIRATORY-PLAN is that we proactively act upon daily data and intervene in a timely manner to limit the severity of any adverse event. We also use this data to provide educational feedback to patients and providers.
 
Cardiocom’s respiratory nurses are experienced in working with a variety of managing physicians such as general practitioners, pulmonologists, and cardiologists. Cardiocom works with the different specialists to customize vital sign monitoring parameters to suit his/her reporting needs.
 
Each day, your patients go through a "Health Check" at home using the Cardiocom CARESTAR. CARESTAR uses a “tree branching” Smart Logic System that is specifically tailored to the patient’s individual disease state and comorbidities. CARESTAR'S Smart Logic System automatically adapts Health Check questions based on each answer entered by the patient. Their symptom data are immediately transmitted and analyzed by our proprietary software. Cardiocom Respiratory Nurse Specialists are alerted by the software when a patient becomes symptomatic or exceeds preset parameters. Predetermined symptom criteria are established by the patient's own physician. CARESTAR can also telemonitor parameters of:

• Temperature
• Peak Flow
• Oxygen Saturation
• Blood Pressure

All Health Check questions can be customized for individual patients based on symptoms and comorbidities. Cardiocom's Respiratory Nurse Specialists call the patient to verify reported symptoms and to gather additional relevant information. If the patient is symptomatic or non-compliant, a detailed "Exception Report" is faxed to the designated physician's office. The patient's physician can then determine the best course of treatment. Physicians are sent only relevant data and are contacted by Cardiocom only when a patient reports a confirmed out-of-bound measurement that requires direct attention. All patients with serious Exception Reports receive follow-up calls by Cardiocom nurses to verify that physician intervention has occurred. In addition to receiving the standard Exception Reports, triggered only when a patient's symptoms fall outside the pre-established parameters, clinicians may request daily symptom updates on patients who are unstable, have recently been hospitalized, or are undergoing medication titration.
 
The specially trained RESPIRATORY-PLAN nurses rely on one-on-one relationships with your patients to deliver targeted care. The nurses use the daily CARESTAR data to provide personalized support for your patients. Patients are assured that changes in their conditions are recognized and that their doctors will be notified. This empowers patients to become more confident and independent in the management of their own health.
